Due to the unique physiological and psychological characteristics of geriatric depression, treatment requires a more cautious approach. In addition to the commonly used pharmacotherapy, psychotherapy, social support, and management of comorbidities are equally important.
Regarding pharmacotherapy, Selective Serotonin Reuptake Inhibitors (SSRIs) are currently the first-line treatment for geriatric depression due to their relatively fewer side effects. However, the dosage should start low and be closely monitored to achieve the best therapeutic effect. Additionally, when stopping the medication, it should be done gradually to avoid withdrawal symptoms. Tricyclic antidepressants, while older antidepressants, are no longer the first-line treatment due to their numerous side effects such as dry mouth, constipation, and palpitations, as well as more drug interactions.
For patients with severe depression or those who do not respond to medication, electroconvulsive therapy (ECT) can be considered as another treatment option. However, psychotherapy, such as cognitive-behavioral therapy, can also effectively improve patients' negative thinking patterns and enhance their ability to cope with stress.
Effective communication skills are fundamental to building trust and understanding patients' needs in clinical care. When a patient hospitalized for depression expresses, "I'm broken all over. My son is unfilial and only wants my property," a nurse can use the clarifying technique, "Are you worried about your health and financial security?" to transform the patient's vague expression into specific concerns. This approach not only allows the nurse to gain a deeper understanding of the patient's psychological state but also makes the patient feel understood and cared for.
If a patient in a ward expresses unmet needs by banging their head against a wall, this is a form of nonverbal communication indicating they may be feeling helpless, seeking attention, or attempting to manipulate others. In such cases, it is essential to provide a clear and consistent environment. For instance, when threatening behaviors occur, all caregivers should respond immediately and calmly, informing the patient of the consequences of continuing such actions, such as temporary restriction of movement. Ensuring a safe environment is paramount. Simultaneously, the patient should be taught to express their needs more appropriately, such as by pressing the call button or making a request to a nurse.
For such patients, communication remains effective, but it must be simple, easy to understand, and accompanied by patient listening, positive responses, and reinforcement of positive behaviors.
In conclusion, communication skills are essential tools for nurses and mental health rehabilitation workers. Through effective communication, we can not only gain a deeper understanding of patients' inner worlds but also provide more empathetic care, helping them regain confidence in life and reintegrate into the community.
In community mental health rehabilitation, communication skills play an equally important role. The goal of rehabilitation is not only to control symptoms but also to improve patients' quality of life and encourage them to participate more actively in social life. Through effective communication, healthcare professionals can assist patients in:
Promoting recovery: Encouraging patients to express their feelings and needs, providing emotional support, and helping patients develop personalized recovery plans.
Improving quality of life: Assisting patients in learning self-care, building relationships, and developing interests to enhance life satisfaction.
Enhancing the ability to participate in treatment plans: Communicating the importance of treatment and encouraging patients to actively participate in the therapeutic process.
Community nurses play a pivotal role in long-term care, assisting families with adjustments and helping clients access healthcare resources. Given their direct interaction with long-term care clients and their families, community nurses possess an in-depth understanding of their needs, challenges, and systemic gaps. This unique perspective enables them to critically analyze the strengths and weaknesses of policies from a professional standpoint. They can actively participate in policy development by bringing the voices of community members to the policy-making level and providing valuable feedback, such as post-implementation evaluations. These first-hand insights assist government agencies in comprehending policy effectiveness and making necessary adjustments.
It is essential to note that the role of a community nurse is not to make decisions for clients but to assist them in making informed choices. Community nurses act more as "navigators" than "decision-makers." They help clients and families find the most suitable solutions, but the ultimate decision-making power rests with the clients and their families.
According to the Long-Term Care Services Act, individuals whose physical or mental disabilities have lasted or are expected to last for six months or more can apply for related long-term care services based on their own or their caregiver's needs.
Community-based long-term care aims to enable individuals who require long-term care to remain in their familiar communities and enjoy a dignified and high-quality life. The core concept is "aging in place," which emphasizes providing various services within the community to enable the elderly or people with disabilities to receive the care they need without having to leave their community for institutional care. Institutional care represents individuals leaving their community and entering a relatively closed environment, which contradicts the principle of "aging in place" emphasized by community-based care.
Home-based hospice care brings medical care to the individual's home, allowing them to spend the final stage of their life in a familiar environment. This fully aligns with the concept of community-based care.
Respite care provides primary caregivers with a temporary break, allowing them to rest and restore their energy so they can continue to provide care. This helps maintain the individual's life at home and is an essential part of community-based care. Respite care is not only applicable to parents caring for young children but also to families caring for disabled elderly or people with disabilities.
Day care provides elderly or disabled individuals with the care, social activities, and rehabilitation services they need during the day, allowing them to spend their days at the center and return home at night. This is also a form of community-based care.
Community mental health rehabilitation can be seen as a concrete implementation of tertiary prevention in the field of mental health.
Mental health prevention is divided into three levels, each targeting a different stage of mental illness with the aim of reducing the incidence, severity, and disability of the disease.
Primary prevention: Focuses on preventing the occurrence of illness. Examples include promoting mental health education, providing stress management courses, and improving living environments. For instance, organizing a public health lecture on stress management at a community health center.
Secondary prevention: Targets individuals who have already developed symptoms or are at high risk, aiming for early diagnosis and treatment to prevent disease progression. Examples include aggressive treatment to prevent disease worsening, early diagnosis, early referral, and early treatment.
Tertiary prevention: Provides rehabilitation and support services for individuals who have already developed the disease, aiming to reduce disability and help patients recover their functioning and reintegrate into society. For example, assisting patients in returning to the community and providing potential employment counseling.
