藥物治療方面,選擇性血清素再吸收抑制劑(SSRIs)因副作用較少,是目前老年憂鬱症的一線用藥。但用藥劑量需從低劑量開始,並密切監測,以達到最佳治療效果。此外,停藥時也需循序漸進,以避免不適症狀。三環抗憂鬱劑是較早期的抗憂鬱藥物,但由於副作用較多,如口乾、便秘、心悸等,且與其他藥物有較多交互作用,因此目前已不再是第一線治療選擇。
Regarding pharmacotherapy, Selective Serotonin Reuptake Inhibitors (SSRIs) are currently the first-line treatment for geriatric depression due to their relatively fewer side effects. However, the dosage should start low and be closely monitored to achieve the best therapeutic effect. Additionally, when stopping the medication, it should be done gradually to avoid withdrawal symptoms. Tricyclic antidepressants, while older antidepressants, are no longer the first-line treatment due to their numerous side effects such as dry mouth, constipation, and palpitations, as well as more drug interactions.

對於重度憂鬱症患者或對藥物治療無效者,電痙攣治療(ECT)可作為另一治療選項。然而,心理治療,如認知行為治療,亦能有效改善患者的負面思維模式,提升其應對壓力的能力。
For patients with severe depression or those who do not respond to medication, electroconvulsive therapy (ECT) can be considered as another treatment option. However, psychotherapy, such as cognitive-behavioral therapy, can also effectively improve patients' negative thinking patterns and enhance their ability to cope with stress.

在臨床照護中,有效的溝通技巧是建立信任關係、了解病人需求的基礎。若病人因為憂鬱症住院,且表示:「我全身都壞了,兒子不孝順只想要我的財產而已。」,護理師可以透過「澄清」的技巧:「妳是擔心自己的身體和財產嗎?」,將病人模糊的表達轉化為具體的擔憂,即對身體健康和財產安全的憂慮。這種方式不僅能讓護理師更深入地了解病人的心理狀態,也能讓病人感受到被理解和關心。

My son is unfilial and only wants my property," a nurse can use the clarifying technique, "Are you worried about your health and financial security?" to transform the patient's vague expression into specific concerns. This approach not only allows the nurse to gain a deeper understanding of the patient's psychological state but also makes the patient feel understood and cared for.

If a patient in a ward expresses unmet needs by banging their head against a wall, this is a form of nonverbal communication indicating they may be feeling helpless, seeking attention, or attempting to manipulate others. In such cases, it is essential to provide a clear and consistent environment. For instance, when threatening behaviors occur, all caregivers should respond immediately and calmly, informing the patient of the consequences of continuing such actions, such as temporary restriction of movement. Ensuring a safe environment is paramount. Simultaneously, the patient should be taught to express their needs more appropriately, such as by pressing the call button or making a request to a nurse.

對這樣的病人來說,溝通仍然是有效的,不過需使用簡潔、易懂的語言溝通,以及耐心傾聽、積極回應、強化正向行為。
For such patients, communication remains effective, but it must be simple, easy to understand, and accompanied by patient listening, positive responses, and reinforcement of positive behaviors.

Institutional care represents individuals leaving their community and entering a relatively closed environment, which contradicts the principle of "aging in place" emphasized by community-based care.

在宅臨終安寧照護
